Hello,everyone. May I have your attention please? I'd like to talk a little about the survey sheets. They are the two pages you have in I your hands.
Our survey sheets are made up of two parts — the question page and the answer page. First,please make sure that you have both of them. The answer page is where you are supposed to write your answers. You don't have to write anything on the question page. Before;you start to answer these questions,you need to put down your name,student card number and the number of your class in their places on the answer page. Remember to write your personal information clearly so that it's easy for others to read. We won’t count your sheet if we can't read your name,number or your class. If you forget or just don't know your student card number,there's a list here on the blackboard,and you can come up to the front to check for it. If there's anything you’re not sure about,please put up your hand. If anyone needs a pencil,I've got some on the teachers’ desk. You can have one. And finally,you have forty minutes to complete the survey.
